Meningitis Response Form
Students are not required to receive the meningococcal vaccine, but they are required to complete this form.
When you click the submit button your web browser will send an email to the Weigel Health Center with your response.
Medical Releases
Medical Release To Weigel
To request that medical records, such as immunizations, be sent from another location to Weigel.
Medical Release From Weigel
To release medical information from Weigel or another location, New York State requires that you provide written consent to do so. Information being released from the Weigel Health Center can be mailed or faxed (716-878-6727). Please expect at least 24–48 hours for processing once the record release has been received.
To request records to be printed by Weigel Health Center, the first 10 pages are free. Every page thereafter is 25 cents per page. Records will not be released until payment for copied pages is received in full.
Payment must be either a personal check (photo ID required) or money order only, made out to Weigel Health Center.
Health Insurance
Health Insurance Requirements
All full-time, and part-time students are recommended to have insurance (either your own, or under a parent/guardian).
- Health insurance is needed for off-campus services: emergency room, urgent care, prescriptions, x-rays, physical therapy, vision, dental services.
- Not all insurances are covered outside of your residential area.
- It is the student's responsibility to contact their insurance carrier to determine if their insurance covers outside services in the Buffalo area.
- Insurances not covered in the Buffalo area: Health First, Metro Plus.

Insurance Cards
- You must provide Weigel Health Center with your insurance card.
- Your insurance card or a photocopy you provide, will be scanned into your chart.
- Your card is needed for outside services, such as lab work, referrals, x-rays, emergency room, etc.
Providing Weigel with Your Insurance Card
Drop Your Card Off at the Center
Bring your insurance card (or a photocopy of both the front and the back of your insurance card) into the Weigel Health Center.
Fax Your Card*
A photocopy of the front and back of your insurance card may be faxed to Weigel Health Center at (716) 878-6727.
*Be sure to write your banner number on the photocopy of your card.
Email Your Card
A copy of the front and back of the card may also be emailed to weigel@buffalostate.edu
Be sure to include your banner number in the subject area of the email.
