Immunizations
Tetanus, measles, mumps, and rubella vaccines; and Tuberculosis tests (PPD) are available at Weigel Health Center at no charge.

The New Student Health Form
New York State Public Health Law 2165 requires college students to show proof of immunity to measles, mumps, and rubella.*
The New Student Health Form must be:
- Completed by all new students.
- Signed by a health care provider or submitted as an official record from a doctor or school to be accepted as proof of immunizations.
- On file at Weigel Health Center.
Note: Students who do not fulfill this requirement will be prevented from registering for classes.
*Students born before January 1, 1957, are exempt from this requirement. Please show proof of age at the Weigel Health Center. Proof of military service also temporarily fulfills this requirement.
Submitting Your Form
The forms must be completed, printed, then faxed, mailed, or emailed back to Weigel Health Center.

New York State Public Health Law requires that students enrolled for at least six semester hours or the equivalent per semester, or at least four semester hours per quarter, must complete the Meningitis Response Form (although, you are not required to receive the vaccine.)
About Meningitis
Certain students are at increased risk for meningococcal disease, a potentially fatal bacterial infection commonly referred to as meningitis. Freshmen living in dorms are found to have a six-fold increased risk for the disease. Students may consider vaccination to reduce risk. Contact your primary doctor or your local Department of Health to obtain this immunization.
It is recommended that college students, particularly those living in dorms, learn more about meningitis and vaccination.
- Know how meningitis is spread.
- Know the symptoms (often mistaken for the flu).
- Know when to seek medical help.
- Know about the vaccine that helps prevent meningitis.
