FLU SEASON IS COMING - BUFFALO STATE FLU SHOT SCHEDULE
Flu vaccines will be available for faculty and staff members on Wednesday, September 20, and Thursday, October 12, between 11:00 a.m. and 1:00 p.m. in Butler Library 210.
No appointment is necessary. The following health insurance plans will be accepted: Blue Cross Blue Shield of WNY, Independent Health, Blue Choice, MVP, and Medicare Part B. Employees without a participating insurance plan will be charged $30 for the flu shot.
Participation and acceptance of any insurance is not a guarantee of benefits or payment in full. Co-payments or a deductible may apply. Please call your insurance provider’s member services department with questions regarding coverage.
Empire Plan enrollees may obtain their flu shot from an Empire Plan participating pharmacy without copay or at their doctor’s office (participating provider) for their usual co-pay should they wish to do so.
Please call Employee Benefits at 878-4821 or the member services department of your insurance company with questions regarding your health insurance coverage. The phone number should be on your insurance card.
Weigel Health Center-Information.
Please submit documentation of immunizations with proof of 2 Measles, 1 Mumps, and 1 Rubella vaccine, or 2 MMR vaccines.
Students are not required to receive the meningococcal vaccine, but they are required to fill out this online Meningitis Response form.
Students do NOT need to submit a Physical or Medical Health Records to WHC.
Please Provide Weigel Health Center with your Insurance Card.
This will be used for any outside services provided, such as Lab work, referals, Xrays.
Bring your insurance card in to Weigel, OR you can bring a PHOTOCOPY of the front AND back of your insurance card. Your insurance card or the photocopy of your insurance card will be scanned into your chart.
A photocopy of the front and back of your insurance card can be faxed to Weigel Health Center,

Make an appointment for your Sports Physical. Please don't wait for the last minute as the schedule fills up quickly.
Make sure you bring your:
- Tetanus information (If you are due you can get the vaccine at Weigel)
- Medical clearances for injuries or illnesses within the past 5 years.
A Sports Physical is required. It must be completed by a Weigel Health Center provider. Please arrive 10 minutes before your appointment to complete physical form, or your appointment may be rescheduled.

If student is participating in a Buffalo State sport, at time of appointment, please submit to Weigel, ducumention of current Tetanus vaccine (TD or TDAP), within the last 10 years) .
Student athelete at time of appointment, please submit to Weigel, a Medical Clearence, if within the past 5 years you have had an injury, surgery, or major illness, from the Medical Doctor that treated you.
